• Bring a set of pertinent records that Dr. Liegner can keep. You should also keep your own set of records.

• There will be a reservation fee of $100 applied to the visit. This will be refundable if notice of cancellation is provided with at least 2 of our business days' notice.

• Initial evaluations often take at least 2 hours, but may take up to 6 hours. Please plan accordingly.

• Dr. Liegner's practice is fee-for-service. A HCFA 1500 form will be provided to patients which they can submit to their insurer for reimbursement to them.

• Initial evaluations and first follow-up visits are recorded for accuracy and for transcription purposes. If you are not comfortable with this, we suggest you seek care elsewhere.
